First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation.

Please assist us by providing the following information at the time of your consultation:

  • A list of medications you are presently taking.
  • If you have dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the consultation visit.

X-Rays

If your dentist has taken x-rays, they may send them to our office or send them with you to your appointment. Not all dentists send x-rays because our facility will always take digital x-rays in addition to the x-rays from your general dentist.










 

Please alert the office if you have a medical condition that may require a Pre-Medication prior to treatment:

  • Rheumatic Fever
  • Mitral Valve Prolapse
  • Heart Murmur
  • Congestive Heart Failure
  • Heart Disease
  • Angina
  • Joint Replacements (Knee/Hip)
  • Cancer
  • Type I Diabetes Insulin Dependent
  • HIV Infections
  • Hemophilia
  • Malnourishment
  • Immunosupression Caused by Drug or Radiation Treatment
  • Rheumatoid Arthritis
  • Systemic Lupus
  • Other Medical Conditions that Cause Patient to be Immunocompromised or Immunosupressed)
  • Or any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
